signed off by me if you are submitting any course additions forms
to the curriculum Committee
PROCEDURE
1. Download
and complete a
copy of the
Library Course Addition Survey Form
2. Send this
hard copy form, a copy of your
course addition forms (filled out), and
the course
outline to Jeri Resto (619-660-4423).
