A&R Forms

Welcome to the Admissions and Records forms depot. The following steps ensure timely processing of your submitted form.

Step 1:

Download the appropriate form. All forms are in PDF format.

Step 2:

Complete the form on your computer, once you have filled in all available fields please print the form.

Step 3:

Using Black or Blue ink please sign and date the form.

Step 4:

Submit the form using one of the three methods below:

Please note: If you are a part of a categorical program (DSPS, EOPS, CalWorks) it is required that you fill out the Program Verification form below and submit with your petition for reinstatement of BOG or enrollment priority. 

(Petitions for enrollment priority are now being accepted up until November 28th)
