Emergency Assistance Questionnaire

The Emergency Assistance Program

 Developed with fundraising from The Foundation for Grossmont & Cuyamaca Colleges and contributions of various community leaders in the great  East County and San Diego areas. For this reason Cuyamaca College can provide those students who are referred with non-recurring emergencies selected funding to allow them to remain in school when faced with unexpected financial hardships. As funding is limited, not all requests can be approved and it is important that you are specific and detailed when answering the on-line application questions and when providing the required documentation to verify your emergency expenses.

Please read the sections below.
After you read each section, check the box next to that section. Checking each box will acknowledge that you have read and understand the information provided.

ONLY after you have read all sections of the questionnaire will you be able to ACCESS the on-line Dreamkeepers Application.

Eligible Expenses

Gas, Food, Textbooks (First two weeks)
Utilities (Past Due with verification), Bus Pass

Rent (Past Due with verification), Medical/Dental, Unexpected Automobile Repair,              Child Care

Ineligible Expenses (but not limited to)

Tuitions, Fees, Legal Fees, Loan Payments,
Tobacco, Alcohol, Normal Bills/Rent, Insurance Bills, Expected Auto Repair
     Expenses/Normanl Wear, Apartment/House Rental Down payment

Cable/Internet, Cell Phone Bill, Entertainment Expenses

Eligibility Requirements

I understand I must be enrolled in a minimum of 3 units at Cuyamaca College.

I understand my current GPA must be 2.0 (minimum)/satisfactory academic progress          for all completed course work.

I understand I must be seeking a certificate, degree or transfer objective.


 

Application Preparation

I understand that I must FIRST complete the FATV Money Management Course and
     send the completion notice to the Financial Aid & Scholarship Office (A300).

I understand SECOND that I must complete the Emergency Assistance Budget 
     Planning Worksheet and turn it into the Financial Aid & Scholarship Office (A300).

I understand THIRD that I must prepare all documents showing "extreme personal/
     financial emergency" consisting of nonrecurring expenses.

Document ALL needs being requested. Make sure your documentation is complete.

I will provide a Utility Past Due Bill with a turn-off, cut-off, or shut-off type of
     notice
.

I will provide an Eviction Notice, a 3-Day Notice, or a Quit Notice along with my
     rental agreement.

I will provide medical documentation and understand a request for unexpected
     emergency medical treatment requires an estimate from a Doctor's or Dentist's
     office and it must be signed by the Doctor or Dentist or a staff member from
     their office who is authorized to sign off on an estimate.

I will provide an completed estimate for unexpected Automobile repair requests.

I will provide an estimate for unexpected Child Care expenses.

Please contact the Financial Aid & Scholarship Office (619) 660-4537 to make an appointment. The office is located in Building A-300 at the Student Services Center. Walk-in appointments are available but very limited so we encourage you to call to make an appointment.
