Faculty & Staff Referral Form - Student Technology Request
Thank you for making a referral on behalf of a student. The purpose of this form is to provide faculty and staff a way to refer students who need technology assistance. Please note that completing this form on behalf of the student does not guarantee the student will receive the technology. The library will process requests on a first come first served basis basis while technology devices are available.
PLEASE SUBMIT ONE APPLICATION PER STUDENT. Once the form is successfully submitted, a library staff member will contact the student within 3 business days to follow-up on whether the requested technology is available for pick-up.
IMPORTANT: Only CURRENTLY enrolled students in courses at Cuyamaca College are eligible to borrow technology devices. Devices are available to borrow and must be returned at the end of the semester.
