TB Risk Assessment Questionnaire
Very Important Information Regarding Assessment, versus TST and Blood Testing
We are currently performing TB Risk Assessment Questionnaire's free of charge by appointment
for students, please call 619-660-4200 to schedule an appointment, you can also contact
your health care provider and have your health provider complete the Risk Assessment
Questionnaire.
Please note if you were born outside of the USA, you will require further testing
not available at Health and Wellness Services, and will need to see your own Primary
Care Provider or Nursing Services can provide you with local community clinics that
perform additional testing if you are uninsured at a fee for service charge.
TB Risk Assessments are done only in person and not over the phone.
Please note the TB Risk Questionnaire is a screening process for active TB, if TB
Skin Testing is recommended, this is a 48-72 hour process where a skin test is delivered
and you must return in 48-72 hours to have the test read. This service is available
for current students and is free of charge if you qualify.
TST's are done only on MONDAY and TUESDAY, so that you may return to have your skin
test read by an RN.
